We are aware that there is a new NHS App - "NHS Digital" - this app is only for NHS England and does not apply in the rest of the UK.  As far as we know there are no non-clinical data sharing policies currently being discussed in Scotland.

The Covid-19 Vaccination Status Scheme is now live and will allow those who have been vaccinated to request proof of vaccine. Only those planning to travel to a country or territory where a record of vaccination status is needed as an entry requirement should download the record or request it.  Please visit the Further Information section, Coronavirus Vaccination for further information.

Please do not attend the surgery to make an appointment, our doors are locked and only patients with pre-booked appointments are allowed access.

New Patient Registration

At present due to covid restrictions we are unable to accept any local interpractice transfers.  If you have just moved into our practice area and would like to register with us, please complete and sign a           GPR REGISTRATION FORM to request registration at the practice.  The full set of forms required to be completed for registering with the Practice are under "New Patients",  "Registration Forms".  We routinely ask patients for some form of identification, for example photo identity such as your passport or driving licence and/or proof of address on a recent utilities or council tax bill.

If you have a Medical Card (Form GP4) this would also be helpful.  Please complete all mandatory sections of the GPR form as this will help us to quickly obtain your medical records from your previous practice.

You will also be asked to complete a Patient Health Check Questionnaire to give us some key information about your past medical history. If for any reason your application is declined (this is very unlikely), we will write to you to explain the reason.

You have the right to express a preference to receive services from a particular clinician or group of clinicians within the Team, generally or in relation to a particular condition.  If you express a preference, then we will record that preference in your medical records.

Your registration will be with the practice as a whole rather than with an individually named GP. However, you may request to see any doctor, and indeed it can be better if you can build a relationship with a particular doctor.  You will understand that all the doctors are busy and that your preferred doctor may be fully booked on the day you wish to see him/her, but we will do our best to find an appointment to suit your needs.

Change of Address

Should you or any member of your family change your name, address, marital status or telephone number  PLEASE REMEMBER TO NOTIFY US  as soon as possible.
